Hazel, a teenager with thyroid cancer, meets Augustus, an osteosarcoma survivor, at a cancer support group, and they quickly bond over their shared experiences and a favorite book, “An Imperial Affliction.” They develop a deep connection, exchanging books and sharing their fears and hopes, leading Augustus to use his Make-A-Wish Foundation wish to take Hazel to Amsterdam to meet the reclusive author, Peter Van Houten. Despite a medical emergency that initially delays the trip, Hazel, Augustus, and Hazel's mother travel to Amsterdam, where they are disappointed by Van Houten's rude and drunken behavior but find solace in each other and share a romantic night. Augustus then reveals that his cancer has returned and is terminal, leading them to return home where his health rapidly deteriorates, culminating in a pre-funeral service where Hazel and their friend Isaac deliver eulogies. After Augustus's death, Hazel discovers a letter he wrote to Van Houten, revealing his profound love for her and his acceptance of his fate, leaving Hazel to reflect on their powerful, albeit brief, love story.
