Adam and Lawrence Gordon awaken chained in a bathroom, realizing they are pawns in a deadly game orchestrated by the Jigsaw Killer, while flashbacks reveal Detective Tapp's obsessive investigation into Jigsaw, leading to his partner's death and Lawrence's implication in the case. As the game progresses, Lawrence discovers his family is being held hostage by Zep, an orderly from his hospital, who is forcing him to complete his task, while Adam remembers being hired by Tapp to photograph Lawrence, leading to a confrontation between the two men. With time running out, Lawrence receives a call from his wife, who is fighting Zep, while Tapp arrives at Lawrence's house and confronts Zep, prompting Lawrence to saw off his own foot to reach a gun and shoot Zep, only for Adam to bludgeon Zep to death. The true Jigsaw Killer, John Kramer, who was disguised as a dead body in the bathroom the entire time, then reveals himself, seals Adam in the bathroom, and leaves Lawrence to crawl away, bleeding profusely.
