Kirsty Cotton, traumatized by her previous encounter with the Cenobites, wakes in the Malahide Institute, where the enigmatic Doctor Malahide studies her case and the supernatural. Malahide, obsessed with the Lament Configuration, secretly resurrects Kirsty's stepmother, Julia Cotton, using the life force of his patients, while Kirsty and Malahide's assistant, Kyle Macrae, uncover his dark experiments. Kirsty and Kyle infiltrate Malahide's house, where Julia, now fully regenerated, murders Kyle, and Malahide, revealing his true intentions, brings the puzzle-obsessed teenager Tiffany to the house to open a gateway to Hell. Kirsty, Tiffany, and Malahide are drawn into Hell, a labyrinthine dimension ruled by the entity Leviathan, where Kirsty confronts her resurrected uncle Frank Cotton and her father Larry Cotton, who are fused together.
