Trevor Gooding, suffering memory loss and hallucinations after a car accident that killed his pregnant wife, Kirsty, is haunted by visions of Pinhead and experiences disturbing apparitions of murders. As Detective Lange investigates Kirsty's disappearance, Trevor's visions intensify, revealing fragmented memories and a mysterious puzzle box, leading him to confess to the murders of Gwen and Tawny. Pinhead reveals to Trevor at the morgue that Trevor is dead, his body one of five souls Kirsty sacrificed to escape the Cenobites, as Kirsty, alive and orchestrating his torment, identifies his corpse. Kirsty receives the puzzle box as part of her inheritance, and as she leaves, the box, now with Lange, begins to open, implying the Cenobites' continued influence.
