Milton, a man escaped from Hell, pursues cult leader Jonah King, who murdered his daughter and abducted his infant granddaughter for a ritual sacrifice. He teams up with Piper, a waitress, and together they battle King's cultists and the mysterious Accountant, an agent of Hell sent to retrieve Milton. Milton reveals his past to Piper, explaining his escape from Hell and his desperate need to save his granddaughter before the three-day window closes. With the help of his old friend Webster, Milton and Piper confront King at his ritual site, where Milton kills King and rescues his granddaughter, before willingly returning to Hell with the Accountant.
